For a cash-pay or private-pay specialty practice, the clearest differences between Sagapixel and Fill The Lobby are how each charges and what each reports. We publish a 30-day Proof Pilot for one flat fee of $2,500, with the ad accounts in the practice's name and a report on bookings and what each cost. Sagapixel's About page describes a client-set budget spent on agency hours for SEO and social media, and a fee of 10% to 22% of ad spend for Google and Facebook ads.
We are Fill The Lobby, so read this with that in mind. Everything here about Sagapixel comes from its own pages, checked on 16 October 2026. "We found no" means nothing published on the pages listed under Sources.

What Sagapixel says it offers
Its home page says it works with healthcare brands on SEO, AI optimization, social media and paid ads together.
Price: one flat fee, or agency hours plus a percentage of ad spend
Our Proof Pilot is 30 days for one flat fee of $2,500, whatever you spend on ads: "One fee, no retainer behind it. Thirty days is the whole commitment." Ad spend goes from the practice's own accounts straight to Google and Meta, and during the pilot we never touch it or mark it up. After 30 days, the long-term price is set by clinician count and given in writing before the practice decides.
Sagapixel's About page describes two models. For SEO and social, "Our clients establish a budget and we use it towards agency hours." For Google and Facebook ads, "We charge a percentage of spend ranging from 10%-22% depending on your ad spend," with "costs for the creative and landing pages, as needed." A PPC pricing post on its site gives a different range: 15% to 22% of ad spend.
Its terms of use call the site's pricing and scope "illustrative and subject to change without notice," and say services are governed by a separate written agreement. We found no published setup fee, minimum budget, set price for creative and landing pages, notice period or fixed-price trial (checked 16 October 2026). Ask for each in writing.
What gets reported: bookings, or hours and tasks
Our pilot ends with a week-four report: what was booked, where each booking came from and what each cost. We're measured on booked appointments.
Sagapixel's About page says clients can see its timesheets: what's being done, by whom, how long it took and what it cost. For ads, its PPC page lists call tracking, and its Meta Ads page lists performance reporting. We found no published report format listing bookings by source with the cost of each (checked 16 October 2026).
Hours and tasks show the work. Bookings by source, each with a cost, show what the spend turned into. Ask Sagapixel for a sample report; ours is the week-four report described on the pilot page.
After the click: calls, follow-up and reactivation
We run eight services as one system. Ads bring the patients; the other seven stop them leaking on the way to the calendar: a missed call, a thin Google profile, an inquiry nobody followed up. Missed-call recovery, follow-up, reactivation and reviews are all in scope.
Sagapixel's SEO page includes managing reviews and the Google Business Profile within local SEO. We found no service on its site for answering or recovering calls, following up with inquiries, or reactivating past patients (checked 16 October 2026). Its terms of use list "your staff's handling of leads" among the factors outside its control.
For a cash-pay practice, an unanswered call is a click already paid for. Ask who picks up when the ads make the phone ring.
Who owns the ad accounts, and what you keep
With us, ad accounts, data, pages and history are set up in the practice's name from day one. A practice that walks away after the pilot keeps everything built.
Sagapixel's web design page says the client owns "the site, the content, and the data." Its privacy policy says it acts as a service provider for data in "a client's advertising accounts." We found no published statement on whose name the Google Ads and Meta accounts are in, or on what a client keeps beyond the website when it leaves (checked 16 October 2026).
Get in writing whose name each account is in before any agency builds in it.
Specialty focus, and the rules that come with it
Each specialty we serve has a page: private-pay therapy, psychiatry, ketamine and Spravato, IOP and PHP, concierge and DPC, functional medicine, weight loss and GLP-1 and gastroenterology. Every tactic we sell runs first on Shifa Therapy and Counsel Clinic, the two platforms our founders own and run, with their own money.
We found no pages in Sagapixel's sitemap for psychiatry, ketamine and Spravato, IOP and PHP, concierge and DPC, functional medicine or gastroenterology (checked 16 October 2026).
These specialties carry board advertising rules. Our compliance page applies to every client: a BAA signed before any patient data moves; no PHI in ad platforms; patient lists never uploaded for targeting; ads reviewed against APA, ACA, NASW and state board advertising rules; no patient testimonials and no outcome claims.
Sagapixel's privacy policy says it works under a BAA where it handles PHI for a covered entity, and its PPC page offers a server-side tracking setup for HIPAA. We found no published statement on checking ads against board advertising rules, or on uploading patient lists to ad platforms (checked 16 October 2026). That doesn't mean it has no policy, so ask. Neither of us is your compliance officer: its terms call its HIPAA and board-rule content general commentary, and we're a marketing company.
Questions to ask Sagapixel before you sign
- Price. What is the fee in dollars at my ad budget? What will creative and landing pages cost? Is there a setup fee or minimum?
- Term. Can I read the service agreement first? What is the notice period?
- Ad spend. Is it billed from my card straight to Google and Meta, or through you?
- Ownership. Whose name are the Google Ads and Meta accounts in? If I leave, do I keep the accounts, landing pages, creative, history and tracking numbers?
- Reporting. Will reports show booked appointments by source and the cost of each?
- Calls and follow-up. Who answers the calls the ads produce? What happens to a missed call, or an inquiry that doesn't book?
- Data. Is a BAA signed before any patient data moves? Is any patient list uploaded to ad platforms? Are my ads checked against my board's rules?
Ask us the same questions. Our answers are on the pilot and compliance pages.
When Fill The Lobby isn't the right fit
We're the wrong choice for hospitals and health systems; pharma, device and life-sciences brands; and practices that mostly bill insurance and want volume.
Try the 30-day Proof Pilot: $2,500 flat, with the accounts in your name. Or start with a free audit, or book a 30-minute call.
